Requesting a rescue is simple. Create a new topic (preferrably using the name of the dungeon and the floor number you are trapped in as the title), then post your wonder mail details in the message. Note, I've added boards for each week, so you should create the topic in the appropriate board.
Anyone can come do the rescue when they feel like it, but points will only be awarded within 4 days of the rescue's first time of posting. That means that if you want to earn points, you will have to be fairly quick about rescuing people!

Edit: I forgot to mention the special characters that appear in the codes. You can either copy the ones here or use the corresponding text as shown below:

Key

♀: (F)
... = (...)

Note: The ... symbol should be typed as (...) to avoid confusion.

HOW TO RESCUE SOMEONE
I won't describe how to input a rescue code for PMD in here - you'll have to go figure that out yourself.

What I WILL describe is how you should go about rescuing them using the forums. First of all, if you are going to attempt rescuing them (and are attempting to be the first to do so) then you should leave a message on their rescue topic letting them know that your rescue team is on the job. Even if someone else has posted their team's intentions first, that doesn't mean you can't get in there and beat them to it!

What's important though is that we know that someone is working on it. If I see people trying to rescue others I may delay the intervention of Aeon Sword long enough for people to complete their rescues, but if you don't let us know you're trying, you might miss out. Naturally, when you complete the rescue, you can make another post with the rescue code. They can then respond with a thank you mail code if they are nice. If you are abandoning a rescue (and the person hasn't been rescued yet), you should also make a post letting everyone know.

Don't make a post if:
  • The player has already been rescued and 4 days have passed (no more points can be earned after that)
  • The player has already been rescued and you're about to attempt a rescue (you can post once you succeed though)
  • You want to post something that isn't either a rescue attempt notification, a rescue code, or a thank you mail (if you were rescued)

Remember, you can still get points if you post your rescue code after the player has been rescued, provided it's within the 4 day time limit. After that, I will lock the topics to make sure people don't forget and post anyway.
